Form 1214 – Withholding from Interest and Dividends

Purpose

Form 1214 is filed by financial institutions (banks, brokers) reporting all withholding tax deducted from interest, dividends, and capital gains during the year. Standard withholding rates: interest 15%–25%, dividends 25%–30%, capital gains 25%.
